2016-2017 Registration Forms
It is time to enroll your child/children in the 2016 – 2017 Faith Formation Program at Our Lady of Lourdes.We offer Faith Formation for children in grades 1 through 9. Please read through the descriptions of each program explained below.
Our Lady of Lourdes offers two different Programs for your child’s 1st through 6th grade Faith Formation; Traditional Classroom Model and Family Faith Formation.
In the Traditional Classroom Model, children in Grades 1 – 6 gather on a weekly basis in a regular classroom setting. Classes meet, once per week, on a Tuesday or Wednesday afternoon from 4-5:15pm. Children and Catechists (teachers) gather in Pollard Hall at 4:00pm for a brief Prayer Service, and then go to their classrooms. The Traditional Classroom Model calendar is based on the Mountainside Public School calendar.
In the Family Faith Formation Program, children in Grades 1 – 6 and their parents meet once a month, on Sunday evening, from 5-7:15pm. Families gather in the Church at 5:00PM for Mass. Attendance at the 5:00PM Mass is a mandatory part of this program. Following the Mass, parents remain in the Church and children go to grade-level classrooms. Parents participate in adult faith formation so that they are equipped to lead their family in learning the Catholic Faith. After the classroom activities, children re-join their parents for announcements and closing prayer. Families are given instructions and lessons to be completed at home. These lessons should be completed as a family and take approximately 45 minutes to 1 hour to complete. The completed lessons must be returned the following session.
Confirmation readiness begins in Grade 7. For children in Grades 7 and 8, The 7th and 8th graders will meet approximately twice per month, and will also be required to attend 4 out of 5 mini retreats on Sunday evenings. A calendar of dates will be provided. Students in both 7th and 8th grade are expected to complete Community Service Hours as well. Classes for Grades 7 and 8 will be offered on Sunday mornings from 10:30-11:45 and Tuesday evenings from 7-8:15.
Grade 9 Confirmation students attend monthly Sunday sessions beginning at the 11:30am Mass and ending at approximately 2:00pm. They too are required to complete Community Service Hours. There is also a Confirmation Retreat for 9th graders.
The success of each of these Programs depends on volunteers. We encourage all parishioners to use their gifts in service of the Church’s educational mission. Please complete and return the Faith Formation Registration Form(s), Emergency Form and Volunteer Form, along with the registration fees to the Faith Formation Office. Specific questions regarding the program for Grades 1 – 8 can be directed to Fran Michetti, Coordinator of Faith Formation, at firstname.lastname@example.org. The $50 late fee will be imposed on all currently enrolled families that register after May 20, 2015. Please note that children will be placed in classes on a first come first served basis and only after all outstanding payments, fees and forms are received. Requests for tuition deferrment due to financial hardship must be made in writing and submitted to the faith formation office. All matters will be kept confidential.
If your child is new to our program, please provide us with a copy of his/her Baptismal certificate. In addition, if your child is transferring from another religious education program, please provide a letter from the parish you have left. Also, if your child received Sacraments at another Church, please give us a copy of those certificates.
