2021-2022 Registration Forms

Registration Information

It is time to enroll your child/children in the 2021 – 2022 Faith Formation Program at Our Lady of Lourdes. We offer Faith Formation for children in grades 1 through 9. Please read through the descriptions of each program explained below. There have been a few changes to next year's programs.

Our Lady of Lourdes offers two different Programs for your child’s 1st through 6th grade Faith Formation: Hybrid Classroom Model and Family Faith Formation.

Carefully read the requirements for each program before making a decision about which one would be best for your family. We will not allow families to switch to Family Faith once the year has started.

In the Hybrid Classroom Model, children in Grades 1 – 6 will begin the school year by completing assignments on Google Classroom and attending an in-person class one time per month.  In-person classes meet, one time per month, on a Tuesday or Wednesday afternoon from 4-5:15PM.  Children and Catechists (teachers) gather in Pollard Hall at 4:00PM for a mini lesson and opening prayer, and then go to their classrooms.  A calendar will be released with the dates when Grades 1-3 and Grades 4-6 will attend each month. We will re-evaluate as the year progresses, and determine when it will be safe to return to full-in person weekly classes. When we return to full in-person weekly classes, we will discontinue the assignments on Google Classroom.

In the Family Faith Formation Program, children in Grades 1 – 6 and their parents must attend 2 Sunday retreats during Advent and Lent from 12:30 until 3PM. At 12:30, families will congregate in the Church near the altar for an opening prayer service, followed by lessons and activities. Home lessons are assigned for each child on a monthly basis and should be completed as a family. Home assignments for each lesson will be posted in google classroom. Children will meet with the director one time per month on Zoom for 5 months for a half hour session on Sundays, grades 1-2 at 1PM, grades 3-4 at 1:30PM, and grades 5-6 at 2PM

**Families who choose this program must be committed to attending the two family retreats and children must be present for the live Zooms.

The preparation for Sacraments of First Reconciliation and First Holy Eucharist  begins in Grade 1 and is a two year process that ends with the Sacraments taking place in Grade 2. In addition to regular Faith Formation classes in Grade 2, there are mandatory family retreats along with other requirements scheduled before each Sacrament.

Confirmation preparation begins in Grade 7.  7th and 8th graders will be required to complete monthly assignments and community service activities on Google Classroom, and attend a live in-person class one time per month on a Tuesday from 7:00PM-8:15PM. We will continue to re-evaluate the program as COVID restrictions change and may add additional in-person classes.

Grade 9 Confirmation students attend monthly Sunday sessions beginning at the 11:30am Mass and ending at approximately 2:00pm.  They, too, are required to complete Community Service Hours.  There is also a Confirmation Retreat for 9th graders as well as a Confirmation rehearsal.

The success of each of these Programs depends on volunteers.  We encourage all parishioners to use their gifts in service of the Church’s educational mission.  If you would like more information about becoming a volunteer in our Faith Formation Programs, please call the Faith Formation Office at 908-233-1777 or email at reled@ollmountainside.org.

Please complete and return the Faith Formation Registration Form(s), Emergency Form and Volunteer Form, along with the registration fees to the Faith Formation Office or to the parish rectory.  Specific questions regarding the program for Grades 1 – 8 can be directed to Fran Michetti, Director of Faith Formation, at fmichetti@ollmountainside.org. For Grade 9 questions, contact Kevin Donahue the Youth Minister, at youthmin@ollmountainside.org. Be sure to label all registrations, "Faith Formation," before dropping off.

A $50 late fee will be imposed on registrations received after May 14, 2020.  The late fee does not apply to families that are new to our program. Please note that children will be placed in classes on a first come first served basis and only after all outstanding payments, fees and forms are received. Requests for tuition deferment due to financial hardship must be made in writing and submitted to the Faith Formation Office. All matters will be kept confidential.

If your child is new to our program, please provide us with a copy of his/her Baptismal certificate. In addition, if your child is transferring from another religious education program, please provide a letter from the parish you have left. Also, if your child received Sacraments at another Church, please give us a copy of those certificates. All families in our program must be registered parishioners, so please also fill out the Parish Registration/Census form.
